We embrace each Hospice of Nursing Placement patient with care, comfort and dignity. Patients are assigned to a Primary Nurse who takes over coordination of care and tends to the emotional and spiritual needs of the patient and their family during this difficult time.
Your care is our concern.
The Primary Nurse relieves the patient and their family of the time consuming task of coordinating care and other related services, allowing the family to spend more time with each other. The Primary Nurse also knows the patient’s right when it comes to hospice coverage from Medicare, Medicaid, and private insurers, ensuring that equipment, supplies, medications and other services are provided.
Wherever you are, we will be there.
Each patient’s Primary Nurse ensures that palliative care is provided whether the patient is residing at home, a nursing home, long-term care facility, or even a hospital.
